SOLDIER ON ADMISSIONS
Our mission at Soldier On is to offer all eligible veterans a way to move forward, to reclaim their lives with integrity, dignity and hope. We do this with a continuum of care that includes immediate short-term housing, as well as permanent supported housing, meals, and clothing. Beyond those basic needs, we also provide treatment/recovery from addictions, access to medical services, counseling, and career building services.
If you or someone you know could benefit from the Soldier On program, the admission process can start by completing the Referral/ Level of Care Assistant Form (Part 1&2). This form can be completed from this link and emailed from this webpage or copied and faxed/mailed to Soldier On. Also, please fill out this release of information.

It is recommended that while you are waiting for your referral to be processed that you locate as many of these documents as you can:
* DD-214
* VA eligibility card
* Birth certificate
* Medical benefits card
Any or all of this documentation will be helpful to have in your possession when you arrive at Soldier On.
