Expand Community Outreach: Providing services not only to current Soldier On residents but to all veterans in the community, broadening the impact of these services.
The Soldier On Healthcare Navigation and Telehealth services provide critical support for veterans, helping them access and navigate healthcare resources, improve mental health, and manage their care from a place of comfort and convenience.
Healthcare Navigation: Soldier On’s Healthcare Navigation services are designed to assist veterans in overcoming healthcare-related barriers. These services are available to all veterans, including those who are not yet utilizing Soldier On's other programs. The focus is on helping veterans navigate complex healthcare systems, ensuring they receive the care they need in an organized and timely manner.
Key Components of Healthcare Navigation
Health Insurance Assistance: Helping veterans understand and manage their health insurance options, including eligibility, enrollment, and coverage details.
Dental Insurance Support: Assisting with obtaining and managing dental insurance and related services.
Provider Communication: Helping veterans communicate with their healthcare providers to ensure they receive proper care and follow-up.
Individualized Healthcare Navigation Assessment: Conducting assessments with veterans to identify their healthcare needs and goals and creating a personalized plan to address them.
Through individualized planning, Soldier On works with each veteran to tackle the specific steps needed to reach their healthcare goals, ensuring they can navigate systems with confidence and clarity.
Telehealth
Telehealth services, in collaboration with Let’s Talk Interactive (LTI), enable Soldier On to provide HIPAA-compliant, remote mental health support to veterans. This platform expands access to mental health counseling and other healthcare services, offering a more flexible and convenient option for veterans.
Key Components of Telehealth:
HIPAA-Compliant Telehealth Platform: A secure platform that ensures the privacy and confidentiality of veterans’ health information during virtual sessions.
Enhanced Diagnostic Ability: The LTI kiosk features technology that allows mental health professionals to adjust the camera for a better view of the veteran’s facial expressions. This helps clinicians more accurately assess micro-expressions, leading to better insights into emotional and mental health.
Veteran Comfort and Control: Many veterans report feeling more relaxed and comfortable during telehealth sessions compared to traditional in-person visits. Being in their own environment can make them feel more in control and reduce the stress often associated with attending in-person appointments.
Use Beyond Mental Health: In addition to telehealth services, the LTI kiosks are used for:
Admissions: Facilitating veteran enrollment and intake processes.
Meetings with Outside Agencies: Connecting with external partners and agencies to coordinate veteran care and services.
Veteran-Centered Approach: Soldier On’s use of telehealth allows for greater access to care, especially for veterans who may face mobility challenges, live in remote areas, or prefer the convenience of receiving care from home. By leveraging the technology to bring services directly to veterans, Soldier On ensures that mental health and other healthcare needs are met without the need for travel or in-person visits.
By integrating Healthcare Navigation and Telehealth, Soldier On is meeting the healthcare needs of veterans in a more comprehensive, flexible, and accessible manner, ensuring they receive the care they need, when and where they need it.
